Three people is not an accident of staffing. It means the person who examined you at intake is the person deciding whether the second injection is worth doing, and the physical therapist working on your movement has spoken to the chiropractor who adjusted you — not read a note, spoken.
Larger clinics have more capacity. Smaller ones have fewer handoffs, and most pain that fails to resolve fails at a handoff.
Diagnosis Comes Before Treatment
Your first appointment is an examination, not a procedure.
History, physical examination, and imaging only where the findings call for it. The reason for that order is that pain is often generated somewhere other than where it hurts. A lumbar disc can present as hip pain with no back pain at all. A facet joint can present as a headache. An L5 nerve root can present as a weak foot.
Treat where it hurts without establishing why and you get temporary relief on a repeating schedule. That is the pattern we are trying to break, and it is why nobody here books you for an injection before an examination has happened.
What We Treat Most in Southwest Denver
Low back pain. Sorting discogenic pain from facet-mediated pain from sacroiliac pain is most of the work. They present similarly and respond to different things. Where exam and imaging agree on a level, a diagnostic medial branch block or transforaminal epidural both identifies the source and treats it.
Sciatica and leg pain. A symptom, not a diagnosis. Lumbar disc, spinal stenosis, piriformis and SI joint problems all produce it, and treating them identically is why sciatica so often drags on. We localise before we treat.
Neck pain and headache. Headaches that start at the base of the skull are frequently managed as migraine for years. We test the upper cervical joints that reproduce them first.
Joint pain. Hip, knee and shoulder — and the question of whether the joint is the problem or the spine is referring into it.
Conservative Care Is the Default
Most pain treated here never needs a needle. Physical therapy with Ben Mannisio, chiropractic with Dr Montgomery, activity modification and time resolve the majority of it, and all of that is available in this building without a new referral.
Where an interventional option is warranted, it is available too: epidural steroid injection, medial branch block and radiofrequency ablation, sacroiliac joint injection, peripheral joint injection. Each is diagnostic as well as therapeutic — the response tells us whether we identified the right structure.
If we do not think a procedure will help you, we will say so.
