Non-surgical pain care on Grant Street, just off 120th Avenue. This is our largest clinic — eleven clinicians including a physician, two chiropractors and two physical therapists — and it functions as the hub for the north Denver metro.

A Physician On Site

Matthew Gray, MD practises at our Northglenn clinic. That is worth stating plainly because it is not the case at every pain clinic, ours included: most are led by physician assistants and nurse practitioners working under medical direction, which is entirely appropriate for the large majority of pain problems.

But some cases need a physician in the room rather than on the end of a phone — complex medication histories, diagnostic uncertainty, patients who have already been through two or three other practices. At Northglenn that is available in the building.

Two Chiropractors, Two Physical Therapists

The practical effect of that depth is that conservative care is not rationed. When the examination says physical therapy twice a week for six weeks, that is schedulable rather than aspirational, and it happens in the same building as the diagnosis.

Diagnosis First

Your first appointment is an examination. History, physical examination, imaging only where the findings call for it.

The reason for that order: pain is routinely generated somewhere other than where it is felt. A lumbar disc presenting as hip pain. A cervical facet joint presenting as a headache. A nerve root presenting as a weak foot with no back pain at all. Treat the site rather than the source and you get relief that expires on schedule.

What We See Across the North Metro

Low back pain, and the work of separating discogenic from facet-mediated from sacroiliac pain — they present alike and respond to different things.

Sciatica and radiating leg pain, where the job is localising the level before treating rather than after.

Neck pain and cervicogenic headache, including the headaches that have been managed as migraine for years without anyone examining the upper cervical spine producing them.

Work injuries. A substantial share of this clinic's caseload arrives through workers' compensation from the warehousing, distribution and trades employers across Adams County. Those cases need documentation as much as treatment, and both are handled here.

Joint pain — hip, knee and shoulder, and the question of whether the joint or the spine is responsible.

Interventional Care When It Is Indicated

Epidural steroid injection, medial branch block and radiofrequency ablation, sacroiliac joint injection, peripheral joint injection.

Each answers a diagnostic question as well as treating a symptom. A block that works identifies the structure responsible. A block that does nothing rules it out. Both outcomes are useful; neither is a wasted appointment.
