The Desk, and What It Does to a Neck

The pattern that comes through this door most often is not an injury at all: a neck and upper back that feel fine in the morning and are unbearable by four, a headache that starts at the base of the skull, and a shoulder blade that aches on one side more than the other. Frequently there is forearm or hand tingling that has been self-diagnosed as carpal tunnel.

That cluster is examinable. The headache is reproducible on testing of the upper cervical joints, which is what distinguishes cervicogenic headache from migraine — and it is treated completely differently. The hand symptoms often have a cervical source that nobody has checked, because everyone stopped at the wrist.

We examine the neck for hand symptoms as a matter of routine. It is not a complicated idea; it is just frequently skipped.

Examination Before Adjustment

Range of motion, orthopaedic testing, neurological screening where symptoms travel into a limb.

The examination separates a joint restriction that manual work will resolve from a disc under load, from a nerve root with genuine neurological findings, from something that should not be adjusted at all. Where imaging is indicated first, we say so.

Techniques

Diversified manual adjustment. Instrument-assisted adjustment where a manual thrust is not wanted. Drop-table technique. Flexion-distraction for disc-related low back pain. Soft-tissue and myofascial work — high value for accumulated postural load. Therapeutic exercise, kept short enough to be done.
