For certain presentations — persistent myofascial pain, some headache patterns, neuropathic pain that has not responded to conventional approaches — it is a genuinely useful option, and having it in the same building means it can be added to a plan rather than requiring a referral to a practice that has never seen your notes.

It is one tool among several, not an alternative to a diagnosis. You still get examined first.

Diagnosis First

The first appointment is an examination — history, physical examination, imaging where the findings call for it.

Pain is often generated by a structure other than the one that hurts. A cervical facet joint producing a headache. A lumbar disc referring into the hip. A nerve root presenting as a weak foot. Treat the site rather than the source and the relief has an expiry date.

Low back pain, and the work of separating disc from facet joint from sacroiliac.

Interventional Options

When the examination and imaging agree on a structure: epidural steroid injection, medial branch block and radiofrequency ablation, sacroiliac joint injection, peripheral joint injection and trigger point injection.

Each answers a diagnostic question as well as treating a symptom. A block that relieves your pain identifies what is responsible; one that does nothing rules a structure out. Both are useful.

If a procedure is unlikely to help, we will say so rather than schedule it.
