Diagnosis Comes First

A first appointment is an examination. History, physical examination, imaging where the findings justify it — and imaging can be ordered here rather than sending you back down the interstate for it.

The order is deliberate. Pain is frequently produced by a structure other than the one that hurts: a lumbar disc referring into the hip, an upper cervical joint producing a headache that behaves like migraine, a nerve root presenting as a weak foot and no back pain at all. Treat the site rather than the source and you get relief with an expiry date.

What Northern Colorado Brings In

Endurance sport. Fort Collins runs, rides and climbs at volume, and the resulting injuries are overuse rather than trauma — sacroiliac and hip pain in runners, position-related neck and low back pain in cyclists, shoulders and elbows in climbers. These build gradually, which is why they get tolerated for months before anyone looks at them.

CSU. A large student and staff population, and with it a steady stream of young athletic injuries and the desk-related neck and upper back pain that comes with graduate work.

Agricultural and trades injuries. Out toward Windsor, Wellington and the Weld County line the caseload changes character entirely: lifting injuries, crush and machinery injuries, and chronic back problems in people whose work does not accommodate rest. These need documentation for workers' compensation as much as they need treatment.

Ordinary degenerative spine pain, still the largest single category, and still requiring the work of separating disc from facet joint from sacroiliac before anything is treated.

Conservative Care Before Anything Else

Most pain treated here never needs an injection. Physical therapy with Megan Kane, chiropractic with Dr Scudder, load management and time resolve the majority — and all of it is in this building, so there is no second referral and no second intake.

When the exam and imaging do agree on a structure, the interventional options are epidural steroid injection, medial branch block and radiofrequency ablation, sacroiliac joint injection and peripheral joint injection. Each is diagnostic as well as therapeutic — the response tells us whether the right structure was identified.

If we do not expect a procedure to help, we will say so rather than book it.
