What that does not tell you is what your Aetna plan covers, because Aetna is not one product. An employer HMO, an open-access PPO, a marketplace plan and a Medicare Advantage plan can all carry the Aetna name and behave differently on the same visit.
So the useful answer is never a promise, it is a check. Call with your card in hand, or request an appointment online, and our team reads your plan: whether Chiropractic, Physical Therapy, Acupuncture or Pain Management is covered under it, what your deductible has left to run, what a visit is likely to cost you, and whether anything needs authorization first.
One thing worth knowing before you call: a Meritain Health card is an Aetna card. Meritain administers self-funded employer plans on the Aetna network, and it appears on our Arizona payer list in its own right. If your card says Meritain and you were looking for Aetna, you are in the right place.
Referrals. Mountain View Pain Center does not require one for any service line. Some Aetna HMO products do require your primary care physician to have one on file before they will pay a specialist claim, and open-access and PPO products generally do not. This is a plan question rather than a clinic question, and it is one of the first things our team checks.
Visit limits. Rehabilitative benefits on commercial plans commonly carry an annual cap on the number of Chiropractic or Physical Therapy visits, counted per calendar year and shared across providers. Where that applies we will tell you the number before your plan of care is built, so the visits are spent on what matters rather than used up early.
Prior authorization. Interventional procedures and advanced imaging are the two places authorization usually comes up. Our team submits it and tracks it, and you should expect to be told what has been submitted and what it is waiting on rather than having to chase it yourself.

Bring your insurance card and a photo ID, any imaging you already have with the report that goes with it, and a list of what you take. If a car accident is involved, bring your claim number and your adjuster’s name.
