A large share of the headaches we see are not migraines at all. Pain that starts at the base of the skull, sits on one side, and worsens with a long drive or a long day at a screen usually comes from the upper cervical joints. That distinction matters because the treatment is completely different: cervicogenic headache responds to manual treatment of the neck, while a true migraine needs a different approach altogether. The examination is designed to tell them apart rather than assume.
Low back pain that stays in the back is a different problem from low back pain that travels down the leg. The first usually responds to manual treatment and a loading program. The second means a nerve root is involved, and the plan changes: imaging when the findings warrant it.
The pain runs from the low back through the buttock and down the back of the leg, and the leg is not where the problem is. A nerve root is being crowded where it leaves the spine, and everything it supplies further down reports the irritation. That is why rubbing the calf does nothing and why the hamstring stretch people try first often makes the next morning worse.
Which root is involved decides what you feel. An L5 root gives numbness across the top of the foot and a weak lift of the toes; an S1 root gives pain into the sole and a calf that tires early on stairs. Establishing that at the first appointment is most of what makes the plan specific rather than generic.
Most sciatica settles without a needle. The work is to stop loading the irritated root and to rebuild what stopped working while you were guarding.

Neck pain that refers into the shoulder blade or down the arm is examined for genuine neurological signs before anyone adjusts anything. Where it is a joint restriction, manual treatment works quickly. Where there is real nerve involvement, we say so and change the plan rather than continuing and hoping.
Joint pain that has not settled after six weeks deserves a proper examination rather than more rest. The Physical Therapy that restores the mechanics happens in the same suite.
You get examined before you get adjusted.
The examination establishes whether your problem is a joint restriction that will respond to manual work, a disc under load, a nerve root with genuine neurological signs, or something that does not belong in a Chiropractic room at all. Range of motion, orthopedic testing, neurological screening of the arm or leg — then a decision.
If the findings suggest imaging first, we will say so and order it rather than adjust and hope.
Diversified manual adjustment. Activator instrument-assisted adjustment for patients who would rather not have a manual thrust — a common request and a reasonable one. Drop-table (Thompson) technique. Soft-tissue and myofascial work. Therapeutic exercise you continue at home between visits. Low back pain and sciatica, the commonest reason people book here. The work is identifying whether the pain is discogenic, facet-mediated, sacroiliac or muscular, because each responds to something different. Facet pain that worsens on extension is not managed like a disc that hurts on forward flexion. Where the examination and imaging agree on a level, a diagnostic medial branch block or transforaminal epidural at the nearest MVPC procedure clinic both confirms the source and treats it.
Neck pain and cervicogenic headache. Headaches that begin at the base of the skull are routinely treated as migraine for years. We look for the upper cervical joints and muscles that reproduce your headache before assuming it is primary. Cervical radiculopathy with arm and hand symptoms is a different problem again, and is treated differently.
Hip, knee and shoulder pain. Joint pain in the west metro skews toward the mountains — Golden and Morrison trailheads, ski season, and the deconditioning that follows a winter of it. We are looking for whether the joint itself is the problem or whether it is referred from the spine, because the answer changes the treatment entirely.
Spine injections and other image-guided procedures are scheduled at the nearest MVPC procedure clinic.
Most pain treated at this clinic never needs a needle. Therapy, manual care, activity modification and time resolve the majority of it.
When it is warranted, the interventional options include epidural steroid injection, medial branch block, radiofrequency ablation and sacroiliac joint injection. These procedures are performed at Northglenn, InterQuest and MVSS, our surgery center, and spine injections are also done at Lowry on scheduled days; evaluation and scheduling happen here. Each is diagnostic as well as therapeutic — if a medial branch block relieves your pain, that tells us which structure is generating it, which determines what comes next.
We will tell you when we do not think a procedure will help. That is a more useful conversation than the alternative.
