So do two names that confuse people at the front desk more than any others: UMR, which administers self-funded employer plans, and Optum, which handles particular benefit categories on behalf of United. Both are on our Colorado list in their own right. If your card carries either name, you have not come to the wrong place.
AARP Medicare Complete, also listed on our Colorado payer grid under its older Secure Horizons and Pacificare names, is a UnitedHealthcare Medicare Advantage product. What none of this tells you is what your plan covers. Call with your card, or request an appointment online, and our team will confirm whether Chiropractic, Physical Therapy, Acupuncture or Pain Management is covered under it, where your deductible stands and what the visit is likely to cost.
Read the back of the card first. The payer name on the front and the claims address on the back do not always match, and the back is the one that decides where a claim goes. Reading both to our team saves the most common delay there is on a United plan.
Referrals. We do not require one for any service line. Some United HMO and Medicare Advantage products want a primary care referral on file for a specialist claim to pay. That is a plan rule, and our team checks it before you are seen.
Visit limits. Rehabilitative benefits commonly carry an annual cap counted across all providers, so visits used elsewhere this year count against the same total. We look the number up before your plan of care is built.
Prior authorization. Interventional procedures and advanced imaging are where it normally applies. Our team submits and tracks it, and you should be told what is outstanding rather than having to ask.
