The first visit is built around telling these apart, because the plan that helps a facet joint does little for a disc, and treating the wrong one wastes the weeks that actually matter.
Because so much of this clinic's caseload is on its feet for a living, the plan usually includes a specific plan for the job itself — how to lift, how to stand, and when a brief change in duties is worth asking for while the low back settles.
Where conservative care has not been enough, the next step follows the examination: a medial branch block and then radiofrequency ablation for a facet joint, an epidural steroid injection where a nerve root is involved, or a joint injection for the sacroiliac joint.
