The pattern we see most often follows travel or a change in activity: a long drive in from out of state, then a sudden jump in walking, golf or yard work after months of a quieter routine elsewhere. It is usually mechanical and it usually responds well, but it is worth having looked at rather than waited out for weeks.
"Low back pain" describes a location, not a single problem, and the usual sources behave differently. A facet joint tends to hurt on standing and arching backward, often worst in the morning. A disc tends to hurt on sitting and forward bending. The sacroiliac joint produces a one-sided ache low down. Muscle guarding can sit on top of any of them and make the pattern harder to read.
The first visit is spent working out which structure is responsible, because the plan that helps a facet joint does very little for a disc, and getting this wrong is what turns a short problem into a long one.
If you are only in Arizona part of the year, say so at the first visit — what can usefully be done in a few weeks is different from a plan built over months, and it is better to design for the time that actually exists. Records travel with you if you ask at the front desk.
Where conservative care has not been enough, the next step follows the examination: a medial branch block and then radiofrequency ablation for a facet joint, an epidural steroid injection where a nerve root is involved, or a joint injection for the sacroiliac joint.
